When try to enter using a different ISP with other IP different than home use I can enter the server without problems, but for almost two weeks I can not enter in the server from my home IP "189.217.76.27". Is my isp baned or someting ?

there are no ban's, try deleting the mod folder and re download

If you got it from steam, id be tempted to uninstall the game completely and try a fresh install.

uninstall game and delete key code from windows reg dont work, 😥 , is a problem with mi ISP 😕 , no more cod4 T_T
