At the end of a 30 day ban is the issuer of the ban going to keep track and restore access to the banned player?
I ask this because IMO I think if a player has earned a ban of any kind it should be up to them to register on the forum and ask to be reinstated.
So actually, if others agree, anyone banned for 30 days would be eligible to come back and play but only if they ask. Maybe they should apologize as well? Just a thought.
This would establish some desire and responsiblity on the part of the problem player to use our servers as well as giving us a means of future contact if there are any more issues.

I think that is a good idea. If they don't take any responsibility for being banned we don't want them back anyway.
