Tell him to nix the nick-laming...for that matter just pick a name and stick with it. Constantly changing names gives me the impression he's trying to hide something.
And of course if he promises to adhere to DOU's rules...especially when clan members are not around the server.
Don't forget to point out that IF his ban is lifted he will be "one and done" as far as offenses go and that it will include complaints from non-clan members.

According my expreience in this kind of cases, rarely a person give apologies.
Because he did, and he was frank and honest, I think we should give it a try...
As Cyggy said, I would require him to maintain a single nickname, (which will be easier to control) apart, obviously, respect the rules.
May be a good idea you talk with him in TS.

I played against this player many times. I had an impression that he likes to kill close. Just before he was banned I spectated on him. Like Mr_Head wrote, he "spent a lot of time lying in prone position" (it was a map DM-DarkCity). Because he stayed all that time in a one spot, I didn't notice nothing bad more in his playing.
I agree also with Cyg, that player who constantly changes names gives the impression he's trying to hide something.
Therefore, I have an idea.
One player - one name, new rule on this server.
Exception for admins only, who want to spectate. If a player wants to change his name should ask the admin before.
Our server became a place where many players want to have a good time and to meet friends. So, what is the reason to use many names?
Btw. let's give him the chance.

he came on ts the other night explaining what had happened and was confused and all that i told the only thing i can say is make an appeal and natan will get to you so he did its a first step .
Not the first time TA members have pulled this shit.

I've spoke to a few DOU members and didnt realize the concept of having so many names could come off as hiding etc.. I respect all of you and hope I can be given a chance, I hop on the ts every once in a while. I know your in a different time zone, but maybe I can jump in before a match or practice and have a chat. Your appeal has been aproved by the clan. That is by the majority, not by all!
We will grant you permission to play on the server again.
Making your appologies has made a lot of difference in this case.
We have a few request though…
Pick just one name and stick to it. Constantly changing names will give us the impression someone is trying to hide something. You need to play according the DOU sniper rules, also when non DOU members (guests) are around. If you’re not sure about a rule its better to ask First.
You will still have to face a 30 days ban starting from now on. So, the ban will end at July 8th.
Remember you will be watched closely when the 30 day ban has been lifted.
Come to chat with us once in awhile on TS, so we can get more familiar. Lots of guests and members from other clans do also.
I will be on ts later today, probably around 6 pm EST.
Otherwise I see you around on ts some other time.
